Quick answer
What Is a Calprotectin Stool Test?
Calprotectin is a protein released by white blood cells called neutrophils when they move to an inflamed area. When your intestines are inflamed, some of that protein passes into your stool. A calprotectin stool test, also called a faecal calprotectin test, measures how much there is.
A raised level suggests inflammation in the bowel. A normal level makes inflammation a less likely cause of your symptoms. The test does not diagnose a condition by itself. It helps your doctor decide what to look at next, and all it needs is a stool sample.
Who Might Need a Calprotectin Test?
A doctor may suggest this test when digestive symptoms keep coming back or last for several weeks. Common symptoms include diarrhoea, abdominal pain or cramping, bloating, and a frequent, urgent need to open your bowels.
Ongoing digestive symptoms
If diarrhoea, cramps or bloating have lasted for weeks, the test can help show whether inflammation is a likely cause.
Telling IBS from IBD
Irritable bowel syndrome (IBS) and inflammatory bowel disease (IBD) can feel very similar. IBD involves inflammation of the bowel, while IBS usually does not. The result helps your doctor choose the right next step.
Monitoring known IBD
If you already have Crohn's disease or ulcerative colitis, repeat testing helps your doctor see how active the inflammation is and whether your treatment is working.
Deciding whether a colonoscopy is needed
A normal result can help your doctor judge whether more invasive tests are necessary. A raised result usually leads to further investigation, which often includes a colonoscopy.
If You Have Blood in Your Stool or Other Warning Signs
You can book a calprotectin test straight away, whatever your symptoms. If you have blood or mucus in your stool, black tarry stools, unexplained weight loss or ongoing abdominal pain, we also recommend speaking to a doctor about them promptly. You can ask for a free doctor consultation with your booking. If a doctor needs to see you in person, a doctor's home visit costs AED 250.
Medilife's Doctor on Call service can assess many urgent but medically stable needs at your home, hotel or office, 24/7. The typical response time is approximately 30 to 45 minutes across Dubai.
If you have severe abdominal pain with vomiting and a swollen abdomen, heavy bleeding, fainting or difficulty breathing, call 998 for an ambulance or go straight to the nearest hospital emergency department. A calprotectin test is not a substitute for emergency care.

Quick answer
How to Prepare for Your Test
- Tell the nurse about your medicines and supplements. Anti-inflammatory painkillers such as ibuprofen, aspirin and naproxen, and acid-reducing medicines called proton pump inhibitors, can affect calprotectin results.
- Keep taking prescribed medicines unless a doctor tells you to stop.
- No fasting is needed.
- Alcohol and smoking: some doctors advise avoiding both for 24 hours before you provide your sample. Ask the Medilife team if you are unsure.
Understanding Your Calprotectin Result
Your report shows a calprotectin level and the laboratory's reference range. Ranges differ between laboratories and by age. Levels are naturally higher in young children and can be higher in older adults, so your doctor reads the number alongside your symptoms.
Not elevated
- What it usually suggests
- Active inflammation in the intestines is less likely. Your doctor may look at other causes, such as IBS.
Borderline
- What it usually suggests
- Mild inflammation, or an effect from medicines. Your doctor may repeat the test or review your symptoms.
Significantly elevated
- What it usually suggests
- Active inflammation in the intestines. Further tests are usually needed to find the cause.
| Result | What it usually suggests |
|---|---|
| Not elevated | Active inflammation in the intestines is less likely. Your doctor may look at other causes, such as IBS. |
| Borderline | Mild inflammation, or an effect from medicines. Your doctor may repeat the test or review your symptoms. |
| Significantly elevated | Active inflammation in the intestines. Further tests are usually needed to find the cause. |
A raised level has several possible causes, including IBD, gut infections, irritation from anti-inflammatory painkillers, diverticulitis and coeliac disease. Less commonly, it is linked to bowel cancer. A raised result does not confirm any of these. A normal result makes active inflammation less likely, but it does not rule out every bowel condition.

No. It helps your doctor decide whether a colonoscopy is needed, but a colonoscopy is still the way to find the exact cause of a raised result.
A calprotectin test measures a marker of bowel inflammation. Blood tests such as CRP measure inflammation in the body more generally, and stool blood tests look for blood. Your doctor may combine them.
Calprotectin levels are naturally higher in young children, so results need careful interpretation by a doctor. Contact the Medilife team to check whether the visit suits your child.
