When Should a Cortisol Blood Test Be Taken?
Cortisol follows a daily, or circadian, pattern. Levels are generally higher earlier in the day and change as the day progresses. For this reason, the time your blood sample is collected can affect how the result is interpreted.
There is not one collection time that is correct for every cortisol test. The appropriate timing depends on the clinical question, whether an AM, PM or other cortisol measurement has been requested, and the instructions associated with the test.
- 01
Morning (AM) Cortisol Testing
A morning cortisol test measures your blood cortisol during the earlier part of its daily cycle. Morning testing is commonly used when a clinician wants a timed cortisol measurement as part of an assessment of adrenal function.
If you have been given a specific collection time by your doctor or laboratory, follow that instruction. Medilife can arrange your blood collection around the required timing.
- 02
PM and Random Cortisol Testing
Cortisol may also be measured later in the day or at another specified time. A PM or random cortisol measurement is not automatically interchangeable with an AM result because the expected cortisol level changes throughout the day.
Not everyone needs both morning and evening testing. The collection schedule should match the test that has been requested.
What Does a Cortisol Blood Test Measure?
A cortisol blood test measures the concentration of cortisol in a venous blood sample. Cortisol is a glucocorticoid hormone produced by the adrenal glands and is sometimes described as a "stress hormone" because of its role in the body's physiological response to stress.
Cortisol is also involved in metabolism, blood-glucose regulation, blood-pressure regulation and immune activity. Its production is influenced by ACTH, or adrenocorticotropic hormone, which is released by the pituitary gland and stimulates the adrenal glands to produce cortisol.
The hypothalamus, pituitary gland, ACTH and adrenal glands form part of the hormonal control system often called the hypothalamic-pituitary-adrenal, or HPA, axis. A cortisol blood test measures cortisol itself. It does not automatically include ACTH or every other hormone involved in this system.

When May Cortisol Testing Be Relevant?
A cortisol test may be requested when a clinician needs information about cortisol production or adrenal and pituitary function. It can form part of an assessment when cortisol excess or cortisol deficiency is being considered, or when an earlier result or clinical situation requires further investigation.
Testing may be relevant in contexts such as:
a clinician-requested cortisol assessment
investigation of possible cortisol excess
investigation of possible adrenal insufficiency
adrenal or pituitary-related endocrine assessment
follow-up of an earlier abnormal or unexpected result
Conditions associated with abnormal cortisol regulation can include Cushing syndrome, Cushing disease, adrenal insufficiency, Addison disease and secondary adrenal insufficiency, but a cortisol result on its own does not diagnose these conditions.
Symptoms such as fatigue, weakness, weight changes, changes in blood pressure, easy bruising or dizziness can occur for many different reasons. They should not be used alone to decide whether someone has high or low cortisol.
How to Prepare for a Cortisol Blood Test
Preparation depends on the type and timing of the cortisol measurement being performed. Medilife can confirm the collection instructions when your test is arranged.
Do not assume that fasting is always required, or that it is never required. Follow the instructions provided for your specific test.
Tell the relevant healthcare professional about medicines you are taking, particularly steroid or corticosteroid medicines, because some medications can affect cortisol measurements. This can include prescribed steroid treatments in different forms. Do not stop a prescribed medicine simply because you are having a cortisol test unless your doctor has specifically told you to do so.
Recent illness, physical stress, vigorous exercise and other clinical factors may also be relevant when a cortisol result is interpreted.
How Medilife Arranges Your Cortisol Blood Test
Booking is designed to keep the process straightforward. Contact Medilife, tell the team you need a Cortisol Blood Test and provide your preferred collection location and timing. The required test timing and preparation instructions can then be confirmed before collection.
A Medilife healthcare professional collects a venous blood sample at the arranged location. The sample is then processed for laboratory measurement, and you receive a laboratory report containing your cortisol result and the applicable reference information.
If the result or your clinical situation requires further assessment, a healthcare professional may recommend additional evaluation or another type of endocrine test.

Blood Collection at Home, Hotel or Office
Home collection
Arrange your cortisol blood draw at home if you prefer to avoid travelling to a clinic, particularly when the test needs to be collected at a specific time.
Hotel collection
Visitors and business travellers in Dubai can arrange professional blood collection at their hotel.
Office collection
If a workplace appointment is more practical, Medilife can arrange cortisol blood collection at your office in Dubai.
These are collection settings for the same Cortisol Blood Test, not different tests.
Understanding Your Cortisol Test Results
Your report will show the cortisol measurement together with the laboratory's applicable reference information. Cortisol results should be considered in relation to the time the sample was collected, the laboratory method and reference interval, medicines, symptoms and the reason the test was ordered.
For this reason, there is no single universal cortisol number that should be applied to every patient, laboratory and collection time.
A result above or below the laboratory reference interval may indicate that further assessment is appropriate, but one high or low cortisol result does not by itself establish a complete endocrine diagnosis. Depending on the clinical situation, a clinician may consider repeat testing or another investigation.
How Cortisol Blood Testing Differs From Related Tests
Different cortisol and adrenal investigations answer different clinical questions. A routine venous cortisol measurement should not be treated as interchangeable with ACTH testing, urine or saliva cortisol testing, or dynamic endocrine procedures.
- 01
Cortisol and ACTH Testing
Cortisol and ACTH are closely related, but they are different measurements. ACTH stimulates the adrenal glands to produce cortisol, so the two may sometimes be considered together when a clinician is investigating how the pituitary and adrenal glands are functioning.
ACTH is not included in Medilife's AED 250 standalone Cortisol Blood Test unless it is separately ordered and confirmed as an available test.
- 02
Blood, Saliva and Urine Cortisol Testing
A blood cortisol test measures cortisol from a venous blood specimen. Cortisol can also be investigated using saliva or urine in other clinical settings, but these methods have different collection procedures and may be used for different diagnostic questions.
Medilife's offer on this page is specifically the Cortisol Blood Test. Salivary and urinary cortisol testing should not be assumed to be included.
- 03
ACTH Stimulation and Dexamethasone Suppression Tests
An ACTH stimulation test and a dexamethasone suppression test are specialised dynamic endocrine investigations. They are different from taking a single baseline blood sample for cortisol.
A clinician may consider these or other investigations when additional information is required, particularly after an abnormal result or when a specific adrenal or pituitary disorder is being investigated.
Standalone Cortisol Test or Broader Hormone Testing?
If you only need a targeted cortisol measurement, the Medilife Cortisol Blood Test is AED 250.
For people who require a broader set of hormone measurements, Medilife also has a broader hormone package for AED 949. This package includes Cortisol, Prolactin, Testosterone Total and Free, AMH, FSH, LH, Estradiol, Progesterone, SHBG, DHEA-S and 17-OHP.
Standalone Cortisol Blood Test
AED 250
Best suited when cortisol itself is the requested measurement. It uses professional venous blood collection, with timing arranged according to the testing requirement.
Broader Hormone Package
AED 949
Designed for broader multi-marker hormone testing rather than cortisol alone. The package should be chosen because its additional measurements are relevant to the reason for testing, not simply because someone has fatigue, stress or a single unusual cortisol result.
Patients looking for broader hormone assessment may also consider Medilife's Female Hormone Profile, Male Hormone Profile or other individual hormone tests according to the clinical question.
